What do you want at the top of my paper?

Mr. Anaya’s requires that every paper turned in for a grade have the following heading.

Top Right, YOUR NAME; new line; THE TEACHER'S NAME, the teacher that will be grading the work; new line; THE CLASS, Mr. Anaya will accept Computers or Technology; new line; THE DATE, spell out the date; a BLANK LINE; THE TITLE, Centered; BLANK LINE; then you can start your work, an essay or numbered questions.
